Understanding PIP Insurance for Massage Therapy in Oregon

If you've been in a car accident in Oregon, you may have heard terms like “first-party” and “third-party” PIP. These phrases can sound technical, but the difference is actually straightforward once you understand how Oregon's system works — and knowing it can help you start care sooner and avoid unexpected billing surprises.
First-Party PIP: Your Insurance Pays First
In Oregon, every auto insurance policy is required to include Personal Injury Protection (PIP). This coverage helps pay for medical treatment after a car accident, regardless of who caused it.
Because of this system, your own auto insurance typically pays first — even if the other driver was at fault. When we bill your own policy for treatment, that is called a first-party claim. Oregon uses this structure so injured drivers can begin care quickly, without waiting for insurance companies to determine fault.
Oregon requires a minimum of $15,000 in PIP benefits on every auto policy. Benefits can be used for up to two years from the date of the accident, or until the policy limit is reached — whichever comes first.
Two years may sound like a long time, but the dollar amount can move quickly — especially when multiple providers (chiropractic, physical therapy, acupuncture, and massage) are billing simultaneously. Carrying higher-than-minimum PIP limits is one of the simplest ways to protect yourself before an accident ever happens.
Some policies also include a deductible. Because adjusters typically do not share deductible amounts or full policy limits directly with our office — that information goes to you as the policyholder — we strongly encourage patients to review and understand their own coverage details.
Why We Do Not Bill Third-Party Auto Insurance
A third-party claim involves billing the at-fault driver's insurance company for your medical treatment. While this is common in legal cases, third-party auto insurance payments often involve extended review periods, delayed processing, and unpredictable timelines.
To keep your treatment consistent and billing predictable, our clinic works within Oregon's first-party PIP system. If PIP benefits are exhausted, we'll discuss next steps clearly and early — no surprises.
How Auto Claim Billing Works at Hive
We offer convenient online booking for auto injury appointments. After you schedule, you'll receive an email with a secure link to complete your intake form. This form gathers everything we need in one place, including:
Your claim number
The state where the accident occurred
Your adjuster's name, phone number, and email (if available)
Your medical history and a description of your injuries
We ask for at least 24 hours before your appointment to verify your claim. While some benefits can be confirmed through online systems, most require speaking directly with your adjuster. Taking this step ahead of time prevents billing issues later.
Important: In Oregon, massage therapists are not required to have a referral to bill PIP when the accident occurred within the state. This allows patients to begin care promptly without waiting on a doctor's visit first.
If your policy includes a deductible, it's usually best to address that portion up front. Our goal is to reduce financial stress so you can focus fully on healing.
What Happens If PIP Benefits Are Exhausted?
PIP benefits remain available for up to two years from the accident date, but once the dollar limit is reached, billing must transition. Depending on your situation, care may shift to health insurance if we are in-network with your plan, or some patients choose to continue on a self-pay basis. In rare cases, and evaluated on an individual basis, we have worked with attorneys under a Letter of Protection to hold a balance until settlement.
We aim to have these conversations early — not after coverage has already run out.
Common Misunderstandings About Auto Claims
A few things we hear regularly that are worth clarifying:
Many people assume the at-fault driver's insurance pays first — in Oregon, it's your own PIP coverage that leads.
PIP has a dollar limit and may include a deductible, which adjusters won't always disclose to your providers.
Not all accident-related care is automatically covered in full, particularly when multiple clinics are involved.
Monitoring your own coverage — especially when receiving care from several providers simultaneously — is one of the best ways to avoid unexpected balances.
The Bottom Line
First-party means we bill your own insurance. Third-party means billing the other driver's insurance — which we don't do directly.
At Hive, we work within Oregon's first-party PIP system to keep treatment timely and billing predictable. Car accidents are disruptive enough — our goal is to make the administrative side of your care clear and organized so you can focus on recovery.
